VUSC/VYSL/US CLUB TEAMS ONLY: Complete all entries below to request field space at Horse Creek. Use this form to request the use of Horse Creek facilities for additional practice times, scrimmages, or multi-game events involving VYSLVUSC/US Club affiliated teams.

Any request for use involving High School, Adult, or non-VYSL/VUSC/US Club youth teams must be approved by the VYSL Board of Directors and those requests must be made directly to Board and not on this form. This request form may only be used for VYSL/VUSC/US Club affiliated events. Teams in bad standing may not reserve fields for any event.

The appropriate fee must be paid 48 hours prior to field useage. Request made in less than prescribed times will not be honored. If an urgent need arises, contact League or Club officals.
