This change of address notification form may be used by the following individuals:
1) International scholars, staff, and faculty sponsored by Dartmouth College
2) J-1 students who have graduated and are working in the U.S. under Academic Training
3) F-1 students who are outside the U.S. attending an fsa or an lsp.
Current enrolled and active Dartmouth international students who have not yet graduated should NOT use this form, but must notify the International Office of a change in residential address by updating their information in College's BannerStudent system.
F-1 Students currently authorized for F-1 OPT should update their address and employer information using the following separate online form: https://fs8.formsite.com/Rcatmur/form918749511/secure_index.html
Dartmouth College-sponsored non-U.S. citizens present in the U.S. in any immigration status other than F-1 or J-1 students and scholars (for example H-1B or TN workers) are also required to report any change of address directly to the U.S. government, Citizenship & Immigration Services (CIS) within 10 days of moving. There are two ways to do this:

2) File a change of address notice (AR-11 form) by mail. The form can be downloaded here (right-click to open in a new window): http://www.uscis.gov/files/form/ar-11.pdf.
