This form is to be completed by the following groups of Journalism School graduates: October 2011 graduates; February 2012 graduates and May 2012 graduates.

May 2012 graduates must first verify that their diploma applications have been received by checking SSOL to see if your diploma application has been registered (“Your Data” section on the right. Under “Academic Records” you will find a link called “Degree Application Status") If your name is not there, please see this DOS Blog posting for instructions.

October 2011 graduates should have already received their diplomas. February 2012 graduates should receive their diplomas by mid-March.

You are required to complete this form even if you do not plan to participate in our graduation ceremony.

Please note that this includes dual-degree students who are completing the Journalism portion of their degrees. You are eligible to march with your classmates (since all your Journalism requirements are completed) even though you won't receive your actual diploma until you have finished both degrees.
