Data Request Process
Health Statistics Section
In order to request analyzed vital records or analyzed survey data from the Health Statistics Section, this Data Request Form must be completed and submitted. For survey data or vital statistics with small numbers suppressed, completion of this Data Request Form is sufficient. For vital statistics with small numbers included or for record-level vital records data, the Data Use Agreement must also be submitted.
All Colorado vital records are confidential in accordance with Colorado statutes (Section 25-2-117). However, Colorado statutes and Board of Health regulations permit vital records and the confidential information contained therein to be used for bona fide research conducted in the public interest or for tracking reportable conditions or investigating disease outbreaks.
The Office of the State Registrar has the authority to release vital records or confidential information from those records for research purposes provided that the researcher meets all conditions for approval. These conditions are outlined in the Data Use Agreement, and this agreement must be signed and submitted for approval with all relevant documentation.
