I do hereby agree to the clauses of this release: Cooperative Christian Ministry in Higher Education at the University of North Carolina at Charlotte dba UCF (hereinafter “UCF”), is the coordinator of the said trip. The group will depart from the state of North Carolina and remain as a group for the entirety of the trip. The group may participate in various mission activities that include light labor, visiting and working in hospitals and/or clinics, orphanages, and churches. UCF coordinates the entire schedule in cooperation with local missionaries or conference planners. As a participant, I understand my participation in the group activity is voluntary and in no way required by UCF. I understand that as a participant in UCF that I cannot hold UCF liable for any injury to my person or property caused by the negligent conduct of another UCF participant. However, a UCF participant who has negligently caused harm to my person or property may be liable to me. UCF does agree, however, to exercise reasonable care in all its activities. In consideration for being accepted by UCF for participation in any activity for the present calendar year, I being 18 years of age or older, do for myself as participant hereby assume all risk of personal injury, sickness, death, damage, and expense which I may incur as a result of the negligence of UCF during participation in recreation and work activities involved. Further, authorization and permission is hereby given to UCF to furnish necessary transportation, food, and lodging. I further hereby agree to hold harmless and indemnify UCF, its directors, employees, and agents, to the extent legally possible, for any liability sustained by said UCF as a result of the negligent, willful and intentional acts of said participant, including expenses incurred attendant thereto. I authorize the trip coordinator if I am unable to do so, to consent to any necessary examination, anesthetic, medical diagnosis, surgery, or treatment and/or hospital care rendered to me under the general or special supervision and on the advice of any physician or surgeon licensed to practice medicine by the state or country in which they practice, during the duration of the said trip. I understand that as a citizen of the United States of America or any other country that I am subject to the laws of the country that I travel in during the said trip, and therefore will exercise legal responsibility. Recent medical and dental exams have ensured that I am in good health. I agree further to carry appropriate health insurance documents and bring an adequate supply of all prescription and other necessary medications, as well as personal hygiene items as needed. I agree to drink only bottled or purified beverages including water or beverages made only with boiled water, if health alerts may have been issued for the water supply. I further agree not to eat ice cubes, raw food, or any dairy products while on this trip—unless I feel confident in the safety of the said item. I will avoid roadside stands and street vendors who sell food. I agree further to swim only in appropriate places and wear clothing which will reduce exposed skin. I understand that insect-borne illness can occur when traveling outside the United States, as well as other hazards pertaining to health that I am aware of, such as: Hepatitis, measles, diptheria, influenza, malaria, animal rabies, and other illnesses commonly found outside the United States. I agree to follow the directions of airport security and of the attendants of the air carrier. I agree to follow all airport guidelines. I understand that some domestic and international airports do not maintain and carry out effective security measures at all times. I further agree to abide by the regulations of U.S. Customs and may be required a duty on items purchased abroad. I agree to pay the total airfare, plus tax and fees, if I decide to cancel this trip. I understand that internationally manufactured pharmaceuticals may be dangerous. Further, I understand that some other nations experience occasional civil unrest, including unofficial roadblocks in all regions. I understand that remaining calm, using common sense, and departing quickly in such a circumstance is the recommended action. Further, I understand that violent crime such as murder, carjacking, armed robberies, kidnapping, and break-ins contribute to security issues outside the United States. Some of these crimes, which sometimes occur in daylight hours, appear to be specifically aimed at Americans. I understand that alertness is particularly important as an international traveler. I understand that neither home nor abroad, “safe areas,” do not necessarily exist. I agree to keep valuables at home or if they must be taken, for the sake of myself and for the safety of the greater group, I agree to keep all valuables well hidden. I understand that the U.S. Embassy recommends compliance without resistance if I find myself in a criminal situation. In the event of a loss of passport or theft, I agree to report the incident immediately to the local police, trip coordinator, and nearest consulate, if applicable. I agree not to voluntarily separate myself from the group or designated sub-groups while on this trip. I finally understand that there is to be no consumption or possession of illegal products.
