PLEASE READ BEFORE COMPLETING APPLICATION

Thank you for taking the time to apply for a Caring Heart Rescue dog. Your application will be processed in the order that it was received.

The following questionnaire will assist us in understanding what you are looking for in your new rescue dog. Please note that we do not adopt to applicants 1) outside the inner suburbs of the Washington DC Metro area and 2) homes with children under the age of 13.

Notification of status of application

If you are not contacted within 14 days, email CHR at chr-news@caringheartsrescue.org for a status report.

Note: If applying for puppies under 1 year old, we require a dog walking reference, or an adopter home part of the day to ensure the puppy has a way to elimate to continue the housetraining process. This applies to individuals who work outside the home and are gone 8-10 hours with the transportation time included. Thank you for understanding!

History of Pet Ownership Please list current and past pets for the last 10 years. Please include the name, breed, age, whether the dog is still with you or deceased, and if the animal is neutered/spayed. If you have never owned a pet, please provide 2 personal references.

References Please provide the name and telephone numbers of your present/past Veterinarians- Please provide a phone number. Applications without a vet telephone number cannot not be processed. If you have owned previous pets, but not for the last 5+ years, please still list your previous Veterinarian.

Your vet will require approval to speak to a Caring Hearts Rescue Representative. Please confirm that you will call them to give approval.

Groomer Reference

If applying for a long haired breed. Are you prepared to pay someone $60.00 or more, every 7-8 weeks to groom your pet? FYI: Please understand that many of the Asian Breeds require a lot of grooming. Matting can occur between the pads of their paws and throughout their coat very easily. It’s recommended to take the dog in for grooming every 6 to 8 weeks. This could cost $55+ for each visit.

Puppies can have destructive behavior like chewing on furniture or moldings in your home. It's a stage they go through. Sometimes dogs can show separation issues the first few weeks and chewing or accidents can happen. What would you do if your dog/s exhibits these types of behaviors?

Some of our Rescue dogs have allergies and we've found that keeping them on a certain diet helps prevent allergic reactions. Do you mind keeping the pet on a certain diet such as a raw or prescription diet purchased from a vet, or would you rather have a dog on a regular diet purchased more easily?

What is the name of the dog food your past or current pet is eating? Please, list wet food too if given.

Do you have plans to introduce an infant into your home in the next 10 years or less?

Like people, pets grow old and require additional medical care that can be costly. Young pets can also develop medical conditions on occasion. If you were confronted with a major medical need for your pet at any age, and extensive diagnostics or procedures were needed, what would you consider the maximum reasonable amount to spend if needed? (Please be specific)

We take the time to really evaluate and get to know our dogs to help you find the right fit for your family. List the issues below that would cause you to give up or not adopt a pet. We do not judge your answers as a negative so please don't be afraid to answer.

ADOPTION FEES - Caring Hearts Rescue requests an adoption fee ranging from $550 to $575 for dogs under 1 year old and for extra medical procedures we performed like dentals. Is this acceptable to you? More information can be found on our website explaining the veterinary care provided by CHR. *
