HIGH SCHOOL ATHLETICS PASSPORT

This form should only be completed if you did not complete online registration or are adding an activity to your child's record after you have registered for school. If you have not yet completed online registration for the 17-18 school year, please complete the athletics information in online registration instead of completing this form.
 
Before completing this form, please have your child present.  They will be required to sign the Heads Up Concussion Fact sheet and the Code of Conduct portion of this form.

Does the student athlete have any of the following: *

AUTHORIZATION SLIPS REQUIRED

To participate in a sport a student athlete MUST have the following forms on file in the ACTIVITIES OFFICE:
 
1. A current athletic physical examination
2. A code of conduct consent form, signed by both parents and the student athlete
3. Concussion parent fact sheet, signed by both parents and student athlete
4. Medical release form
5. Consent for Athletic Conditioning, Training, and Health Care Procedures
6. The appropriate usage fee paid.
 
All paperwork should be returned to the OFFICE.  Do NOT return the completed physical examination form to your coach.

ATHLETIC PHYSICAL EXAMINATION

In accordance with the Iowa High School Athletic Association, any student participating in activities is required to have a CURRENT sports physical.  The form must be completely filled out, front and back.  The form MUST be signed and dated by parent/guardian, as well as, signed and dated by the physician performing the examination. Any forms submitted to the Activities Office incomplete will be considered invalid and returned for completion. Physical forms need to be submitted annually (form are only valid for 13 months).
