Adoption Fee: Adoption First Animal Rescue and Adopting Parents have agreed upon the donation to Adoption First Animal Rescue for help with expenses incurred by saving this pet. Adoption Fee schedule is as follows: Puppies under 1 year old - $325; Adults 1-9 years - $300; Seniors (10 years +) - $150 *

I agree to provide care from this day forward to my new pet. This includes adequate food, water, exercise, shelter, veterinary care, and lots of love. INITIAL BELOW *

The pet will not be an outdoor only pet and will be allowed to interact with the family members when home. INITIAL BELOW *

I agree to never sell, trade, or surrender the pet to any agency without contacting Adoption First Animal Rescue. A BREACH IN CONTRACT WILL RESULT IN A $1,000 PENALTY. INITIAL BELOW *

I agree that Adoption First will have first option for reclaiming the pet. INITIAL BELOW *

I agree the adoption fee is non-refundable and there is no reimbursement if the animal is returned. INITIAL BELOW

I agree to never commit animal cruelty or neglect. If at any time I feel I cannot handle the responsibilities of the pet, I will return the pet to Adoption First Animal Rescue. INITIAL BELOW *

I agree to provide a safe collar with rabies tag as well as ID tag. INITIAL BELOW *

I agree to provide all veterinary care needed for the pet including annual check- ups, vaccines, emergency care, heartworm and flea prevention, etc. *

ADOPTER SIGNATURE I agree that all expenses incurred after I take possession of pet will be my sole responsibility and Adoption First Animal Rescue is released of any and all liabilities whether financially or otherwise and I indemnify and hold Adoption First Animal Rescue harmless for any action arising out of the adoption of the pet. I hereby understand that Adoption First Animal Rescue may have no prior knowledge of the pet’s behavior, age, breed, health issues, training, etc. Any information shared by Adoption First Animal Rescue is of own experience with the pet and Adoption First Animal Rescue makes no warranties of any kind concerning pet. DISCLAIMER OF WARRANTIES: ALL PETS ARE ADOPTED “AS IS” AND “WITH ALL FAULTS.” ADOPTION FIRST ANIMAL RESCUE MAKES NO REPRESENTATIONS OR WARRANTIES OF ANY KIND CONCERNING THE PET OR SUITABILITY OF THE PET, EITHER EXPRESS OR IMPLIED AND HEREBY DISCLAIMS ALL WARRANTIES INCLUDING WITHOUT LIMITATION ANY IMPLIED WARRANTIES OR MERCHANTABILITY OR FITNESS FOR A PARTICULAR PURPOSE. If at any point Adoption First Animal Rescue discovers there were inconsistencies within the application, or any type of fraud was committed Adoption First Animal Rescue may take possession of the pet without refunding any of the adoption fees. This agreement shall be governed by the laws of the State of Kentucky and the exclusive forum for any dispute arising under this agreement and/or the adoption of the pet shall be in Kenton County, Kentucky. *
