Site Coordinator Registration Form

Use this form if you are organizing a cleanup for a neighborhood, organization or group.

Coordinator Agreement:I agree to have participants sign in, and to distribute cleanup supplies and "Thank You" coupons to the cleanup volunteers. I acknowledge and assume all responsibility and risk as a result of any volunteers who participate in my cleanup. Keep Greater Milwaukee Beautiful, Inc., its Board of Directors, staff, and volunteers will not be held liable for injury or damage.  I agree to return the volunteer sign-in sheets and any unused cleanup supplies and coupons to the KGMB office after our cleanup. I understand that if I fail to return this information/supplies by June 2 (Friday), 2017, KGMB may not be able to distribute supplies or coupons to myself or my group in the future.  Also, please complete the 2017 Great American Cleanup Wrap Up Report following your event.

Are you interested in borrowing gloves for your cleanup/beautification event? Gloves are available on a first-come, first-serve basis. Maximum: One per volunteer. If gloves are available, they will be included in your kit. We cannot guarantee gloves and ask that you encourage volunteers to bring gloves. Schools do not receive gloves.
Are you interested in the Borrow A Tool (B.A.T.) program for your beautification or cleanup event? City of Milwaukee residents only.  Tools must be picked up from KGMB on the Thursday prior to the event and returned the following Monday. A copy of your Drivers License ID will be required as a deposit.  Schools do not receive tools.

When & Where will you pick up your kit? KGMB is located at 1313 W Mount Vernon Ave Milwaukee WI 53233

Please choose a FRIDAY beginning February 24th when you will come in to pick up your cleanup kit. We are open on Fridays for kit pickup from 9:00am to 6:00pm. Please note we will not be open Friday April 14th or Friday May 26th.  Kit pickup for these dates is on the preceding Thursday (April 13 and May 25 respectively).

If possible, please give us at least ONE WEEK advance notice (7 days) to prepare your kit.

Note: A $25.00 voluntary donation is appreciated for any groups of 25 or more. Thank you!
We appreciate your time and support of the 2015 Great American Cleanup. KGMB relies on our dedicated volunteers to ensure the success of our many programs and activities. Please consider making a voluntary donation. Donations can be collected from volunteers at your cleanup and dropped off at KGMB (1313 W. Mount Vernon Avenue, Milwaukee, WI 53233) upon completion of your event.
