F-1 Optional Practical Training (OPT) Work Authorization Recommendation Request Form

This form is to be used by Dartmouth College-sponsored F-1 students only, to apply for an endorsement for Optional Practical Training (OPT) from their immigration advisors on the current I-20 form.
For all OPT applications, our office will need the following, in addition to this online form:
1) A copy of your current passport ID page, and renewal page if applicable
2) If you entered the U.S. by land, a copy of your most recent I-94 card, front and back.  If you entered the U.S. by air or sea, a copy of your admission stamp and electronic Form I-94 print-out.
For pre and post-graduation OPT, and for STEM OPT extensions, we will also need:
3) A copy of the completed government I-765 form (found online: http://www.uscis.gov/files/form/i-765.pdf
).  This form must be typed as a pdf; we cannot accept copies of hand-written forms.

For STEM OPT extensions only, we will need a letter from your employer. This letter must confirm their job offer to you, or your continuing employment.  It must also confirm that, as required by regulation, the employer agrees to notify our office within 48 hours of your employment termination. You may scan all the required copies into one pdf file and upload it at the end of this form, or you may come to our office and submit paper copies to your immigration advisor.

By submitting this application, you are applying to our office for a RECOMMENDATION for OPT.  You must still submit an application to the U.S. government to receive the actual OPT work card.  See the presentation above for details.

PROGRAM END DATE:

We need to know the date you will complete your degree requirements.  This is the date you finish your last exam, or submit your thesis.  You must confirm this date with your faculty supervisor, or the Registrar's Office.  If you are applying for post-completion OPT, and you fail to complete your degree requirements by the end date you list here, you could lose your legal status in the U.S.

OPT Dates:

We need to know the dates you wish your OPT to begin and end.  If you are applying for post-completion OPT, the start date must be between the day after your program end date, and 60 days later.  For post-completion OPT, the end date will be 12 months after the start date unless you have previously used pre-completion OPT at this degree level.  If you have previously used pre-completion OPT at this degree level, subtract the days already used and calculate the date your OPT will end.

For STEM extension OPT, the start date is the day after the current OPT ends, and the end date will be 17 months after the new start date. 

For OPT cap-gap extensions, the start date is the date you submit this form, and the end date will be October 1st of the current year.
