BLUEcloud MobileStaff Device Registration

SWAN Member Libraries and Information Technology Staff:

Please fill out the survey below as accurately as possible. Multiple surveys may be submitted per library to accommodate additional devices. However, please avoid re-submitting previously registered devices. Devices without an associated agreement/registration form cannot be licensed by SWAN.

In preparation for licensing by SWAN staff, please have your mobile devices set up completely WITH MobileStaff already installed and ready to accept a license.

Licensing Agreement

The following agreement has been established between SWAN and the associated member library (listed below) to assure the usage of the SirsiDynix MobileStaff client is done in such a manner that maintains the security of patron and library data.

Library agrees to keep mobile device secured using at least a 4-digit access pin.
Library agrees to keep mobile device registered and regularly monitored using device’s location settings and a 'FindMy' application or similar.
Library agrees to only allow access to this device by designated library staff. Device will not, at any time, be loaned to patrons or non-library staff.
Library agrees to inform SWAN immediately in the event of a lost or stolen device.
Library agrees to inform SWAN and uninstall/remove completely the MobileStaff application should the device be repurposed, sold, donated, or disposed.
Library agrees to allow SWAN to perform any necessary installation, licensing, and maintenance of the MobileStaff application. Library will not attempt to license additional devices without SWAN’s assistance.
Library agrees to maintain the inventory list below and keep SWAN informed of any changes immediately.
