The Person requesting services from Allstate Pilot Car LLC. Agrees to all terms and service pricing listed on this contracting agreement and has signed electronically and has agreed to this contract. Allstate Pilot Car LLC.  provides Professional Pilot Car Services and are fully insured - Leading and or following your over dimensional load. Lead escort is responsible for many different duties, from confirming routing to warning the public of the presence of a following over dimensional to warning the truck driver of roadside hazards.  Rear escorts generally follow behind the load to warn the public of a leading over dimensional load. We will do our best to ensure the safety of the public and the load and your driver. The truck driver must have a working CB radio and stay in contact with the truck safety escort at all times.

Terms:

The Contracting Agent, Trucking Company or Broker agrees to hold Allstate Pilot Car LLC. And drivers harmless for any damages to equipment, persons and or property involved in the oversize load move. The Contracting Agent, Trucking Company or Broker approves all charges, wait time, no goes, over nights, dead head by signing this agreement. Contracting Agent agrees to pay Allstate Pilot Car LLC in full at the end of service. The Contracting Agent, Trucking Company or Broker agrees that the payment be charged to the credit card listed above unless other arrangements have been made in advance of service commencing. A copy of the invoice and receipt will be faxed, e-mailed or mailed. A deposit of 50 % of the estimated amount of the entire service is due at the time this agreement is signed fo unestablished accounts. A 3.5 percent fee for processing will be added to all credit card purchases. Deposits are none refundable if service is provided.The Contracting Agent, Trucking Company or Broker Agrees to give up the right to dispute all charges to the credit card company listed above. The Contracting Agent, Trucking Company or Broker Agrees that any disputes are to be addressed by Allstate Pilot Car LLC only. If payment is not received as agreed, The Contracting Agent, Trucking Company or Broker agrees to pay five percent per month late fee for any outstanding balance. Invoices over 45 days shall be considered in default and subject to legal action or arbitration to take place in Medina County in the state of Ohio to resolve any conflicts at the purchasers cost. The Contracting Agent, Trucking Company or Broker agrees to pay all fees to collect debt that will take place in North Canton, Ohio arising from service provide by Allstate Pilot Car LLC. You are giving the right to Allstate Pilot Car LLC to stop the oversize load at any time, if there is poor radio communication between the escorts and the truck driver, if the truck driver is careless and is a danger to the public, if the oversize load dimensions are misrepresented, or if the truck driver is uncooperative, or if safety flags signs or warning lights are missing or if the truck driver is breaking the law. Allstate Pilot Car LLC will never let the public knowingly be in danger. Once the oversize load is stopped the trucking company will be notified. If an agreement to move the oversize load in not reached the escort will notify the state police as to the problem. At this point the escort may leave the load your company will be charged for the service that has been completed and dead head returning to the home office along with overnights.
